-The enrollment form will open. You will need to enter information on three tabs...
Personal Details: Last Name, First Name, Address, Date of Birth, Sex, Email Address, Work Phone Number
Emergency Contact: First Name, Last Name, Phone Number
Employer: Employer Name
-There is a disclaimer at the top of the sign up... please disregard it. This sign up can be used by all patients 18 years of age and older.
-Access to My Health Gateway will be provided once processing is complete. For established patients, the process can take up to a week.
For new or prospective patients, processing will take place once your patient status with Western Sierra Medical Clinic is activated... this can take approximately 14 days.
