Western Sierra case managers assist patients when it comes to meeting with a provider or are in need of treatment, but they will also educate patients about health issues, such as diabetes or high-blood pressure.

And case managers will assist with social service needs, if needed. Social service issues often have a tremendous effect on a patient’s health. For example, case managers will help patients find housing, emergency food and clothing within the region and also put them in touch with Nevada County 2-1-1, an information and referral hub that connects people with community, health and other services.
